成医附二院医保病人就诊须知

　　1、我院是省、市、城铁医保定点的三级乙等医院，门槛费为800元（该费用不予报销，不纳入治疗费）。入院时首诊医生根据疾病病情预计住院总费用，按照比例收预交金，其中不涉及手术及贵重耗材的内、外科疾病按总费用的30%收取；涉及手术及使用贵重耗材的内、外科疾病按总费用的50%收取。

　　2、参保人员入院最迟在3个工作日内持本人身份证、医保卡前往住院收费处刷医保卡确认其医保身份后，方可享受医保报销。

　　3、参保人员必须结清上一次住院费用后再到我院刷医保卡住院，若不结清会造成两家医院住院时间交叉重叠，无法在我院正常联网结算；同样到下一家医院住院前也必须先结清我院住院费用。

　　4、省医保特殊门诊病人须持本人身份证、医保卡，挂专科号就诊。

　　5、入院前三日内的15项特殊检查阳性结果可纳入医保报销：X线造影、核磁共振扫描（MRI）、X线计算机体层扫描（CT）、彩色多普勒超声检查、伽马照相、单光子发射计算机断层显像（SPECT）、白细胞残留病灶检测、肿瘤相关抗原测定、动态心电图、动态脑电图、纤维支气管镜检、胃镜、肠镜、组织病理学检查与诊断、经纤支镜防污染采样刷检查。特别提示：请医保住院病人在入院后立即将涉及以上15项特殊检查的阳性报告单、发票交予主管医生办理相关转账手续。

　　Medicare patients notice

　　1. Our hospital is a Class 3 Grade B hospital which is the designated provincial, urban and railway health insurance unit. The threshold fee is RMB 800 Yuan (this fee cannot be reimbursed, or incorporated into the treatment cost). When entering hospital, the doctor who first makes diagnosis for you will predict the total hospitalization expense according to the state of your illness, and collect the deposit at a certain ratio, in which the internal medicine disease and surgical disease which do not involve any surgery and valuable consumables, a 30% of the total expenses should be collected, for those involve surgery and valuable consumables, a 50% of the total expenses should be collected.

　　2. Medicare patients should take their ID cards and Medicare cards and go to the cashier at the hospitalization department to confirm their ID as Medicare personnel by swiping the Medicare card  within 3 working days after admission, and then they can enjoy Medical insurance reimbursement.

　　3. Before entering our hospital for hospitalization by swiping the Medicare card, Medicare patient must pay off the expenses for the last hospitalization, if not so, the hospitalization time of two hospitals will be overlapped, and could not settle accounts normally through the Internet in our hospital. Likewise, before entering next hospital, the hospitalization expenses of our hospital must be paid off first.

　　4. Special outpatients with provincial medical insurance must carry their own ID card, Medicare card, and take specialized registration for seeing doctor.

　　5. Fifteen special Positive test results which examined within 3 days since entering hospital could bring into medical insurance for reimbursement, which are: X-ray radiography, MRI, CT, Color Doppler ultrasonography,.Gammagraphy, SPECT, White blood cells residual lesion detection, Tumor related antigen determination, Dynamic electrocardiogram, Fiberoptic bronchoscopy, Gastroscope, Enteroscope, Histopathological examination and diagnosis, The pollution prevention fiber brush mirror sampling inspection. Special notice: Medicare patient should deliver the positive report note of the above 15 special examination and invoice to doctor in charge immediately after hospitalized to go through relevant transferring procedure.
