浙医四院患者的权利和义务

　　患者权利：

　　Patients'Rights

　　1、平等医疗权：凡患者不分性别、国籍、民族、信仰、社会地位和病情轻重，都有获得平等医疗的权利。

　　Right of healthequality: Patients have a right to receive equalmedical care, regardless of gender, nationality, ethnic group, religion, socialstatus and illness severity.

　　2、知情同意权：患者有权了解病情、诊疗措施、风险、疗效及愈后；进行手术、麻醉或镇静、有创操作、使用血浆或血液制品、化疗放疗、临床试验或其他高风险服务时，须获得患者或其代理人的签字同意。

　　Right of informedconsent: Patients have a right to be informed ofstate of illness, diagnosis, risks, effect and prognosis; when patients undergosurgery, anesthesia or sedation, invasive operation,transfusion of plasma or blood products, chemotherapy and radiotherapy,clinical trial or other high-risk-procedures, written consent should beobtained from the patients themselves or their family members/guardians.

　　3、医疗决策参与权：选择就诊医师及相关治疗方案方面，患者有决策参与权。

　　Right ofparticipation in all medical decisions: Patients have a right to participate in selecting doctors and make treatmentplans.

　　4、获得权：患者有权获取法规规定可复印的病历资料及治疗产生的相关费用清单。

　　Right of access topersonal medical documents: Patients have rightsof access to their medical records and list of expenses by law.

　　5、隐私保护权：患者就诊期间的隐私得到医院的尊重及保护。

　　Right of Privacyprotection: Patient privacy is respected andprotected by hospital during the medical care.

　　6、投诉权：患者有权对就诊期间获取的服务提出意见并得到反馈。

　　Rightto make a complaint: Patients have rights to make comments on the services received during themedical care, and request feedback.

　　7、人格权：患者有在就诊期间的人格尊严及其宗教和文化受到尊重的权利。

　　Right ofpersonality: Patients have a right to have theirpersonal dignity, religious belief and culture respected during the medicalcare.

　　8、人身安全和财产保护权：患者有在安全的医疗环境下接受诊疗照护的权利。

　　Right of personaland possession security: Patients haverights to receive diagnosis and treatment in a safe environment.

　　患者义务

　　Patients' Obligations

　　1、患者在就诊过程中有提供真实的个人及健康相关的信息的义务。

　　Patients areobligated to provide accurate personal and health information duringthe medical care.

　　2、患者有配合医务人员的治疗，积极参并做出决策的义务。

　　Patientsare obligated to cooperate during the treatment, and participate indecision-making.

　　3、患者在就诊过程中，有遵守医院相关的规章制度，服从医院工作人员的安排的义务。

　　Patientsare obligated to obey the regulations and comply withthe arrangement of the hospital.

　　4、就诊期间有爱护医院设施设备，服从安排的义务。

　　Patients areobligated to protect public facilities and equipment, and complywith the arrangements of our hospital.

　　5、患者有主动支付治疗过程中产生的相关费用的义务。

　　Patientshare obligated to pay the expenses generated during the treatment.

　　6、我院是教学医院，您在住院时过程中，可能会有学生（医学、护理等）、培训生及进修生在老师的指导下进修相关的教学工作，请您配合。您的病历资料（包括临床、病理、影像等）都有可能用于教学、科研用途。

　　The hospital is a teaching hospital.During your hospitalization, it is possible that students (medical students,nurses), trainees and would make some medical practices under the guidance oftheir teachers. We would appreciate it very much if you could cooperate. Inaddition, your medical records (including clinical, pathological and imagingrecords) may be used for teaching or scientific research.

　　7、住院期间您不得到院外就诊、购药、私自请医师来我院会诊及采取其他治疗手段。

　　Duringhospitalization, no patients are allowed to attend to other hospital, purchasedrugs outside our hospital, privately ask for consultation from other doctorsoutside our hospital, or adopt other treatment without permission.
